Neuropsychiatric symptoms of Parkinson's disease are some of the factors determining the patient's quality of life, but often remain underestimated by clinicians, especially in the prodromal stage, when affective symptoms can be assessed without regard to the neurological disease. Objective: to study the clinical characteristics of affective disorders and their comorbid pathological characteristics, manifesting in the prodromal phase of Parkinson's disease (PD) within 10-15 years before the diagnosis of PD, as well as their effect on adherence to treatment. Materials and methods: 29 patients with diagnosed Parkinson's disease were examined using questionnaires (Toronto alexthymia scale (TAS); Minimult questionnaire; Structured Clinical Interview for DSM-IV Axis II Personality Disorders (SCID-II); Hospital Anxiety and Depression Scale (HADS); Hamilton Scale for Evaluation of Depression (HAM-D)), a clinical psychopathological method with collection of objective information, a neurological examination. Results: premorbid features of cluster C personality disorders were more often diagnosed (48%) in patients with PD. Affective disorders in the prodromal period of PD are represented by the following conditions: dysthymia (13.7%) cyclothymia (6.8%), recurrent depressive disorder (34.4%), single depressive episode (44.8%). Affective disorders in PD are characterized by a late onset, predominance of anxiety, asthenic and apathetic disorders, as well as a high level of alexithymia and somatic symptom disorder.
