Телемедицинские консультации в детской онкологии – гематологии: современный взгляд на актуальность, проблемы и их решения при оказании профильной медицинской помощи
The research r elev anc e is determined by the demand for telemedicine consultations (TMC) as one of health care digitalization pathways in Russian Federation. Telemedicine, regardless of the field, continues to prove its significant role, saving health resources without losing medical care quality. The TMC is regulated by federal level framework. Each year the number of TMC in the «pediatric oncology and hematology» profile has a tendency to increase that allows to provide qualitative specialized patients’ residential medical care and reduce the level of their hospitalization to federal centers.The r esear ch goal: to analyze the relevance of TMC in the field of «pediatric oncology and hematology», as well as to develop proposals for ensuring their sustainable function and high quality in federal and regional clinics.R esear ch methods included analytical, synthesis, content-analysis and classification analysis and legal monitoring. Retrospective analysis of TMC data on pediatric oncology and hematology, including malignant neoplasms of the central nervous system (CNS) in children in 2021 and 2024 was also carried out.The r esults showed that from 2021 to 2024 the number of TMC in the field of «pediatric oncology and hematology» increased more than twice, covering 79 regions of Russian Federation, including new ones. The main reasons for referrals were specific therapy conducting in the regional hospitals and getting a «second opinion». The rapid annual increase for TMC requests has revealed the key problems: lack of staff, high workload, the importance to review both the TMC fees and the overall hospitals’ funding level.The conclusions confirm that demand for TMC in pediatric oncology and hematology is increasing. The focus on CNS tumors is due to both: high mortality and difficulties of diagnosis and treatment. Appeals from the regional physicians are often a request for«second opinion», that indicates the complexity of these clinical cases. Telemedicine continues to face organizational, technical and staff challenges which solution should be a priority for modern healthcare leaders. Finally, proposals to improve the TMC quality and efficiency are presented.
