The paper presents a brief analysis on the quality of cause-specific mortality statistics at old ages in Russia. Two aspects are in focus: the regional differences in approaches for choosing the underlying cause of death and the comparison of Russian coding practice with the practices of some other European countries. The results obtained indicate that there are certain problems with the quality of cause-of-death coding at old ages in Russia. No unified approach to coding deaths as caused by “Senility” is used at a sub-national level. This leads to the distortion of the regional cause-specific mortality structures. Furthermore for many groups of causes Russian death rates are much lower than those in European countries. This peculiarity is caused by the specificity of Russian cause-of-death coding practice which results in underestimating the mortality from certain causes at old ages.

The paper addresses state policy in the area of primary health care payment methods in the system of mandatory health insurance in Russia. The need for the transition from physicians’ visits reimbursement to capitation payment is substantiated. It is argued that the current payment method hinders disease prevention activities, chronic disease management, integration of service delivery as well as a shift of services from inpatient to outpatient settings. The latest regulatory documents on this subject are critically evaluated with the focus on the attempts to combine the capitation method with the fee-for-service one. Alternative approaches aimed at cost containing and strengthening disease prevention are discussed. Versions of fundholding schemes are suggested as well as the ways to mitigate their excessive financial risks. Various options of the pay-for-performance are seen as the necessary addition to the fundholding scheme. Problems of the capitation rate development under the fundholding are reviewed. It is argued that the budget of a fundholder should include only 163 Sheiman Igor M. The methods of planning primary health care utilization should also change – from the current planning of the number of a physician’s visits to the planning of the size of a catchment area in terms of the number of residents served by primary health care providers. The mortality rate from external causes in general and individual causes within this class in Russia is two times higher than similar levels in the developed countries. However, a number of researchers who deal with medical certification of the death, have written about the underestimation of mortality from external causes such as homicides, suicides, alcohol poisonings, etc. and translating them into latent form. In this regard, on the topic of external reasons, an interview was held with leading experts in the field of demography, statistics and medicine. On the basis of studying experts' opinion article describes issues of quantitative and qualitative statistical registration of mortality from external causes in Russia.   According to experts, statistics on external causes must be published on detailed codes of the ICD-10. So, despite the fact that overall levels of mortality from external causes and their types are seen on the basis of the Mortality Tabulation List of ICD-10, but it is often insufficient for a deep and detailed analysis of the mortality. The experts also opined on the reasons for the growth of injuries with undetermined intent, and ways to reduce mortality from these causes, among which the main are changing in the requirements for filling and modification of procedure for issuance of death certificates; legislative securing the position that a beginning of the investigation should serve as a statement, a message about the crime, and not a rule of procedure on the criminal case; cardinal improvement of the system of forensic examination.

On the basis of introduced definition and description of concept of “psychotechnical system” the Client-Centered psychotherapy(CCT) is reviewed and analyzed. Personal characteristics of a CCT specialist, goals, conceptual system and system of practice of CCT, system of interaction between client and psychotherapist are described. It is shown, that this psychotechical view is useful for understanding of process and results of ССT and potentially may be used for analysis of any psychotherapeutic and educational approach.

Medical information systems are a separate class of information systems, specifically developed to improve the efficiency of health care. The purpose of the implementation of health information systems in clinical centers – a comprehensive solution of problems in information support of the delivery of health services, with emphasis on the formalization of business processes in the field of health care, the collection and secure storage of patients personal data, optimal interface solutions for clinicians and nurses, special managing of medications. The main role of the users of medical information system – heads of clinical departments, doctors and nurses. The paper is dedicated to the study of the set of coupled problems with clinical (bedside) diagnoses in medical information systems: effective input, validity and completeness checking, analysis of clinical data taking diagnoses into account. It is common in current generation of medical information systems to input diagnosis as an unstructured text field in natural language with additional selection of some classes from International Classification of Diseases or other universal disease classification. There are standardized guidelines and local conventions for input and change diagnosis text but, as a rule, these conventions are not formalized in medical information systems that leads to above-listed problems. We suggest an effective and easy variant of formalization of extended clinical diagnosis in oncology. The paper proposes a variant of formalizing additional classification of clinical diagnoses, both simple and effective, and the prototype description templates and schemes to enhance the diagnosis for certain diseases in the format JSON, optimization interface standard field "diagnosis" in medical information systems. This variant was introduced as software pilot project in the field of pediatric oncology.
